Welcome to the Malaysian Healthy Diet Online Survey (MHDOS)!

Are you eating healthily? Find out by taking the MHDOS. Completing the MHDOS will provide you with information on how well your diet complies with the Malaysian Dietary Guidelines.

The entire survey may take approximately 15 minutes to complete. We’ll use the answers to calculate your Diet Score which compares your diet against the recommendations of Malaysian Dietary Guidelines. The higher your score, the closer you are to meeting the recommendations. On completion of the survey, you will receive a Diet Score report and some personalised feedback to help you improve your eating habits.

Please note: The Diet Score may not be applicable if you have special dietary needs or restrictions.

How often do you usually eat fruits?

Fruits include fresh fruits, dried fruits and canned fruits. DO NOT include fruit juice.

Then, we'll ask how many servings of fruits you usually have every week. If you usually have 1 apple, 2 bananas and 1 mango during the week, that would be 4 servings. So, slide the bar (shown below) until it says "4".
